Disease Management Programs
Lagniappe Homecare believes in providing the resources and education our patients need to help them recover from an illness or an injury or to manage a chronic disease. We offer qualifying post-acute patients a comprehensive package of educational materials and “tools” called a Care Kit. These kits are equipped with tools specific to a patient’s needs/diagnosis such as a digital scale for a heart failure patient or pulse ox for a post-acute pneumonia patient. Our complete list of disease management Care Kits includes Heath Failure, COPD, Hypertension, Diabetes, Pneumonia, and Balance Training (high fall risk or history of falls). Our highly trained and experienced staff is also available to care for patients with other diagnoses including, but not limited to Alzheimer’s, Cancer, Arthritis, Coronary Artery Disease, or those needing Wound Management.
Stroke and Neuro Rehabilitation
Lagniappe Homecare’s highly trained staff of nurses and therapists will deliver a treatment plan designed by the physician to help the patient regain and maximize independence at home following a stroke or other neurological illness.
Taking numerous medications is called polypharmacy, and it is particularly common among older adults as they are more likely to need multiple medications to manage an array of chronic conditions. Lagniappe nurses and aides help our patients keep track of what can be a dizzying assortment of prescriptions to ensure compliance with their individual medication regimen.
We offer a comprehensive approach to care for a patient’s recovery process following surgery. Whether it’s total hip or knee replacement, CABG, or a complex surgical wound that needs attention post-hospital, our staff of professional nurses and therapists can deliver just what the doctor ordered.
Accidental falls are a leading cause of hospitalization among seniors. Lagniappe Homecare offers a comprehensive in-home assessment of patient mobility and the home environment to insure optimal safety and minimize the risk of a debilitating fall.
